Note: This registration form must be submitted no later than the morning of the registration deadline to the Office of Student Affairs. Original signatures of the student and the advisor are required on this form.
Please note the deadlines for courses changes listed on the Academic Calendar at: http://publichealth.lsuhsc.edu/AcademicCalendar.html. The Course Registration Form is also used to drop or add credits. It must be signed by both student and advisor and returned to the Office of Student Affairs for processing.

Note: This Student Health Form must be submitted to the Office of Student Health Services for processing prior to the first day of class. Louisiana State Law requires submission of these immunization records prior to class attendance.

Note: This Authorization for the Use of Title IV Funds Form must be submitted during the fall registration period. Deliver it to the Office of Student Affairs with your registration form. Thesis Defense Forms
Request for Thesis Defense
Note: This form must be completed and a copy of the Dissertation/Thesis Abstract must be received by the School of Public Health two weeks prior to the defense date. Copies of the Dissertation/Thesis must also be circulated to the examining committee at that time. This form must be submit to the Office of Student Affairs in order to schedule your final defense.

Note: This form must be completed and submitted following the final defense presentation to the Office of Student Affairs.
